Hygiene Hypothesis

  • Prof. David P. Strachan, lecturer in epidemiology at the London School of Hygiene and Tropical Medicine, presented the Hygiene Hypothesis, observing that children with older siblings in the same household had lower rates of hay fever and eczema. This has been expanded to Type 1 Diabetes, Multiple Sclerosis, Depression, and Cancer.

    Children showed improvements in symptoms or markers of allergic diseases if they:
    -were not born Caeserian Section
    -owned a pet
    -attended day care
    -lived in a crowded household
    -were less frequently bathed or had their hands washed
    -were not vaccinated
    -received fewer antibiotics early in life
    -were exposed to bacteria and lived on farms
    -were exposed to farm animals
    -drank unpasteurized milk
    -had no public sewage or public water
    -were infected by viral diseases such as hepatitis A, herpes, or measles
    -were infected by salmonellla, Heliobactor pylori ( in the stomach lining), or Myobacterium –tuberculosis (responsible for tuberculosis)
    -were infected by parasites or parasitic diseases such as malaria, giant roundworms,hookworms, flukes, pinworms, whip worm, or Toxoplasma gondi.

The rise of autoimmune disease and acute lymphoblastic leukemia in young people in the developed world was linked to the Hygiene Hypothesis, as well as Autism. Risk of chronic inflammatory diseases also depends on factors such as diet, pollution, physical activity, obesity, socio-economic factors, and stress.
  • The “Microbial Diversity” hypothesis, proposed by Paolo Matricardi and developed by von Hertzen, holds that diversity of microbes in the gut and other sites is a key factor for priming the immune system, rather than stable colonization with a particular species. Exposure to diverse organisms in early development builds a “database” that allows the immune system to identify harmful agents and normalize once the danger is eliminated. For allergic disease, the most important times for exposure are: early in development; later during pregnancy; and the first few days or months of infancy. Exposure needs to be maintained over a significant period. This fits with evidence that delivery by may be associated with increased allergies, whilst breastfeeding can be protective.
